HMO insurance plans,require you to pick a primary care provider. Please make sure we are listed as your provider before your appointment. Call the plan as soon as you receive your cards if any changes need to be made.

Understanding Your Policy

Most services are covered by insurance but may be subject to a copay, coinsurance, deductibles, and maximum benefits. Some insurances cover limited well and sick visits.

Insurance companies also, charge you a copay for combined office visits. We make every attempt to verify your eligibility, we are unable to verify specific benefits for the services. You will be billed for what your insurance does not cover. Your coverage is a contract between you and your insurance company. It is important that you are familiar with the benefits your insurance plan covers. 

 

Co-Ordination of Benefits

Often insurance companies withhold our payment of a claim when they are waiting to receive a response from you regarding any additional insurance coverage or proof of previous insurance coverage. When you recieve a letter from your insurance company or a phone call from us, please respond prompltly, so that you will not become individually responsible for your bill.

Co-Payments

Co-pays are your contract with your insurance company. They are due on the date of service. If co-pay are not received within the 1st billing cycle,it will be subject to a $10 billing fee. Your insurance requires that a co-pay be collected anytime you present for care.

Payment plans are available for those with financial hardship or are without insurance. Our office does offer State Health Immunization-Vaccine For Children (VFC) program for patients that are 18yrs old and younger, have no insurance or have limited health insurance. Payment plans msut be agreed upon before your appointment.
