Scheduling Hours:  1st appointment of the day starts at 7:40 a.m.  Last appointment of the day 4:20 p.m.

We schedule 20 minute appointments for sick children and 30 minute appointments for well children. Please let the staff know if you think you will need additional time with the doctor.

Our office is closed for Memorial Day, 4th of July, Labor Day, Thanksgiving Day, Christmas Day and New Year's Day. However, one of our doctors is always on call to meet any after hour emergencies.

 

  • We do not have an answering service, you always will speak to a person. One of our providers is on call after hours 24 hours per day, 365 days per year for emergencies after hours.
  • PLEASE USE AFTER HOUR CALLING FOR EMERGENCIES ONLY. Prescription refills are done during regular office hours only. The on-call doctor does not take messages for the office,make or change appointments,or give lab results. 
  • You can NOT LEAVE A MESSAGE for the office on our machine.

 

  • CO-PAYS AND DEDUCTIBLES AMOUNTS are due at check in, for each child seen each time they are seen. As a courtesy, the office will bill your insurance company for all services for each date of service. You are responsible for NON-COVERED services, Coinsurance or DEDUCTIBLES that may be charged in addition to co-pays and payment made on date of services being rendered. A billing fee is added to balances outstanding more than 30 days. Previous balances must have active payment on the account before a new visit can be made. We have the right to reschedule a wellness exam with any account that has an outstanding balance. We accept Master card, Visa, cash, and local checks.
  • **NEW POLICY** ALREADY TAKEN INTO EFFECT: SUSPENSION OF CARE DUE TO NON-PAYMENT ON OUTSTANDING BALANCES OLDER THAN 180 DAYS. Unless your account has been approved by our office manager on a monthly payment plan. 
  • Our office can no longer bill AUTO INSURANCE you may pay at time of service and paperwork will be provided for you to submit to your auto insurance provider directly.     

 

  • For patients with NO INSURANCE, we offer a prompt pay discount; services must be paid in full at the visit. 
  • In the case of DIVORCE or CHILD CUSTODY, the person who brings the child for treatment is responsible for the payment of co-pays, non-covered services, co-insurance and deductibles. We will present a duplicate receipt upon request to facilitate reimbursement. 
  • COMBINED VISITS happen when a child is scheduled for a checkup but other concerns are brought up or the provider discovers them during the visit. If a visit would have been required to address these concerns, then your insurance company may consider these two separate visits and apply co-pays, co-insurances, or deductibles accordingly.
  • MEDICATION REFILLS:

          If you need a refill on your child's medication, we require the parent to call our office to                                                  request a refill WE DO NOT do pharmact refill request.

          IN GENERAL,ANTIBIOTICS CANNOT BE REFILLED WITHOUT FIRST BEING SEEN IN OFFICE BY THE                DOCTORS.

          ADHD MEDICATIONS: ADHD medications must also be requested by a phone call to the                                    office staff and each month it must be picked up in person at our office.

          It is Dr. Ellis policy to know the NAME of the medication and MG that your child takes when                             you request a refill.

NEW STATE LAW EFFECTIVE 7-1-18 ALL CHILDREN WITH  ADHD MEDICATION SCRIPTS REQUIRE AN APPOINTMENT EVERY THREE MONTHS EVEN IF NO MEDICATION CHANGES ARE ANTICIPATED.

  • ADDITIONAL FEES will be charged for physical forms for daycare, school, college, or camp (unless completed at a visit), vaccines forms, letters written by provider, copies of medical records, return check fees, and other services requested. FMLA forms require a visit. 


  • MISSED APPOINTMENTS POLICY: EFFECTIVE 03/01/2018  We make every effort to remind you of your up coming appointment with our reminder system.  It is  very important to keep your phone number and email up to date with our office. With the rising number of "no-shows" or "no courtesy telephone call" prior to a scheduled appointments, We are now forced to charge you a $25.00 for a missed appointment. This fee is for the courtesy reminder system, turned away other patients and keeping staff on the clock.

 

  •  VACCINE POLICY We recommend ALL patients be vaccinated according to the American Academy of Pediatrics endorsed vaccine schedule. All exemptions, whether personal or religious in nature, MUST BE PROVIDED by the Health Department. We offer scheduled appointment regarding any concerns or questions you may have.

  Newborns must begin the standard vaccination schedule by their 2 month well visit or have an                alternate vaccine schedule in place.

 Effective 03/01/2018 we are no longer accepting any new patients/families that do not       vaccinate their child. 

Parents, if you cannot commit to a minimum vaccination standard, or to an alternate schedule then we will no  longer be able to commit as your health care provider. Once a parent notifies our office of their decision to refuse vaccination or on you choose to not vaccinate on an alternate schedule that you and the provider discussed, we will ask you to find a new pediatrician.
